We aim to provide you with
medical care of the highest quality. If you have any comments or suggestions
on how we could improve our services, or if you wish to make a complaint,
please feel free to contact our practice manager.
Computerisation
The practice holds computerised health records and is registered under the Data Protection Act, under which you have statutory rights.
You may be asked to fill in a patient questionnaire in order to complete our records of all patients.
The computer requires good discipline in maintaining accurate records which is to everyone's advantage, and holds the key to many benefits, particularly in preventative
medicine.
Please inform us of any changes to your particulars, address, marital status etc. Confidentiality will remain our prime concern.
Please be prepared to give your computer number or date of birth when contacting the surgery for any reason as this is how your details are obtained from the
computer.
Access to Health Records
You are entitled, by law, to have access to your medical records which were written after 1st November 1991. You must apply in writing and a charge of £10.00, plus any photocopying fees incurred, will be made. The doctor is entitled to withhold any information which they consider could be detrimental to you.
Freedom Of Information – Publication Scheme
The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the ‘classes’ of information
the practice intends to routinely make available. This scheme is available from reception.
Zero Tolerance
We strongly support the NHS policy on zero tolerance. Anyone attending the surgery who abuses the GPs, staff or other patients be it verbally, physically or in any threatening manner whatsoever, will risk removal from the practice list. In extreme cases we may summon the police to remove offenders from the practice premises.
